Sole Proprietorship – Please submit the following documentation:

Partnership – Please submit the following documentation:

  • Dealership partners with current license must submit Affidavit State Form 37964 stating that the dealership will be going out of business.  Affidavit must be signed by all partners & notarized. Please list name of dealership, address & 7 digit dealer number.
  • Current dealer license
  • All metal & interim plates along with registrations
  • New dealership partners must submit a new Application for Dealer Business License - State Form 13215; with the basic documents identified in the New Applications section.

Partner Change - If one partner is coming off the license, submit Affidavit State Form 37964 stating which partner will be removed from license. Affidavit must be signed by all parties involved & notarized. Please list name of dealership, address & 7 digit dealer number.

Corporation/LLC/LLP – Please submit the following documentation:

  • Dealership Corporation with current license must submit notarized letter stating that the dealership will be going out of business.  Notarized letter  must be signed by all officers. Please list name of dealership, address & 7 digit dealer number.
  • Current dealer license
  • All metal & interim plates along with registrations
  • New Corporation must submit a new Application for Dealer Business License - State Form 13215; with the basic documents identified in the New Applications section.

Officer Change - Dealership Corporation with current license must submit notarized letter, signed by all parties involved, stating the minutes of the meeting & changing of officers. Letter must be signed by owner & notarized. Please list name of dealership, address & 7 digit dealer number.
