Welcome to the home page for the Indiana State Department of Health (ISDH) Division of Long Term Care.  The Division of Long Term Care consists of health care facilities licensing and certification programs.  The ISDH has created individual program home pages for the programs.

The Indiana State Department of Health (ISDH) invites all comprehensive long term care facilities to attend the Indiana Long Term Care Leadership Conference scheduled for September 23, 2008 in Indianapolis at the Indiana Convention Center. The topic of the September Leadership Conference is “Emergency Preparedness Planning.”   The ISDH invites you to send two representatives to the conference.  Each facility may select the individuals attending the conference on behalf of their facility. The ISDH recommends individuals such as the facility administrator, director of nursing, or compliance officer attend this conference.  Corporate offices of facilities are also invited to send appropriate representatives.

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.
