Beginning January 14, 2008 accessing the Indiana State Department of Health Nurse Aide Registry and renewal of registration became easier.  The Registry now allows for both paper and electronic renewal of registration for QMAs. Learn more about the changes.

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Reporting Form is only for facilities to notify the Indiana State Department of Health of an unusual occurence. This form is not to be used to file a complaint.
