Small Business applications will no longer be accepted for 2008.  Please begin submitting your applications for 2009 on January, 1st.

There are several ways to apply for the tax credit as a small employer, so explore all of the options listed below before submitting a plan. All documents must be submitted electronically.

There are several ways to apply for the tax credit as a small employer, so explore all of the options listed below before submitting a plan. All documents must be submitted electronically.

  1. If you are already working with a Registered Vendor to implement a worksite wellness program, please notify your Registered Vendor that you would like to apply for the tax credit.  It is the responsibility of the Registered Vendor to submit the name of the small employer they are working with to implement a worksite wellness program.

  2. If you are interested in starting a worksite wellness program and are not sure where to begin, visit our list of Registered Vendors to help you choose a vendor.  A Registered Vendor is a business or organization that directly provides wellness programs to employers as part of their normal business practice.  This term does not include a business or organization that acts in the capacity of a broker, or something similar, for this type of service.  Once you are working with a Registered Vendor, please notify them that you would like to apply for the tax credit.  It is the responsibility of the Registered Vendor to submit the name of the small employer they are working with to implement a worksite wellness program.

  3. If you would like to design your own worksite wellness program without the help of a Registered Vendor, visit the Certification Criteria page to make sure your program meets the criteria.  Once you have designed the program and started the implementation process, your wellness program can be submitted by clicking here. Keep in mind that your wellness program must be submitted in an outline form.  Click here for an example of the outline. Your application should be no more than 5 pages.
