The Diabetes Advisory Council was created to address the ‘Leadership and Management’ key component for Diabetes Prevention and Control Programs as specified in the ancillary documents to the Centers for Disease Control and Prevention grant, Systems-Based Diabetes Prevention and Control Programs (Program Announcement number 03017, grant number: U32/CCU522713-04). One of the model standards for the 'Leadership and Management' key component is to organize and maintain a diabetes advisory group with subgroups as necessary.

The purpose of the Diabetes Advisory Council is to provide the Indiana Diabetes Prevention and Control Program with strategy development, program direction, and support in the implementation and evaluation of specific interventions.

Vision Statement

A state where the public is fully aware of the impact of diabetes; all patients with diabetes are receiving high quality care wherever they are in the state regardless of race, ethnicity, and socioeconomic status; and patients with diabetes enjoy the best quality of life possible.

Mission Statement

  • To increase public awareness of the impact of diabetes;
  • To improve the quality of life for those who are affected by diabetes;
  • To improve the quality of care for patients with diabetes; and
  • To reduce the burdens imposed by diabetes.

The mission will be accomplished through the collaborative efforts of the Indiana State Department of Health, diabetes service organizations, private and public health organizations, health plans, individuals with diabetes, business community, community–based organizations, universities and research entities, community members, and other governmental agencies working toward a common goal.

Committee Descriptions

  1. Data and Surveillance: identify and make accessible diabetes data; create a state health profile; and compile and provide data to organizations for program planning and evaluation.
  2. Diabetes in Minorities: identify minority populations within the state; assist in the development of partnerships to reduce barriers; and promote access to diabetes health care for minority populations.
  3. Education : inform and educate people about diabetes health issues and assure a competent diabetes health care workforce.
  4. Health Services: assess and promote linking people to needed diabetes health services; assess the availability of diabetes health care services to the state's population; inform policy makers of barriers to accessing diabetes services within the state; and to assist communities in developing programs to improve access.
  5. Partnerships: mobilize partnerships and build constituencies who can identify diabetes priorities and create effective solutions; and build statewide partnerships and provide assistance to partners and communities to organize and take action to improve diabetes health in their communities.
  6. Women's Health: serve as a steering committee for Indiana ASIST 2010, a three-year federal grant from Health and Human Services to improve health outcomes for females with diabetes by helping them overcome barriers to accessing health care services and community resources. The grant will support a pilot project in three Indiana counties (Elkhart, Howard, and Lawrence).

The full council meets quarterly at the State Department of Health in Rice Auditorium. All full council meetings are scheduled from 1 - 4 pm on the following dates:

Indiana Diabetes Assessment Congress

The Centers for Disease Control and Prevention (CDC) asked each state to assess the current Diabetes Public Health System using the 10 Essential Public Health Services. On August 18, 2004, the Indiana State Department of Health held the 2004 Indiana Diabetes Assessment Congress in Indianapolis, Indiana.
