About the Program

The Indiana State Department of Health’s Diabetes Prevention and Control Program (DPCP) is funded through the Centers for Disease Control and Prevention, Division of Diabetes Translation.  The DPCP mission is to reduce the burden of diabetes in Indiana through data surveillance, health communications, health systems development, and development and implementation of community interventions and programs. The vision of the DPCP is a  state where the public is fully aware of the impact of diabetes; all patients with diabetes are receiving high quality care wherever they are in the state regardless of race, ethnicity, and socioeconomic status; and patients with diabetes enjoy the best quality of life possible.

The Indiana Women's Diabetes Initiative is formerly the ASIST 2010 Grant.  Office of Women's Health at the Indiana State Department of Health recently received a three-year grant from Health and Human Services to improve health outcomes for females with diabetes. The grant will support a pilot project in three Indiana counties, Elkhart, Howard, and Lawrence. Patient Navigators will assist females with diabetes in successfully managing their diabetes by helping them overcome barriers to accessing health care services and community resources. The DPCP and the Division of Aging at Family and Social Services received a two year grant from the National Council on Aging (NCOA) to implement an evidence-based Chronic Disease Self-Management Program (CDSMP) developed by Stanford University.  "Living a Healthy Life" will be implemented in the sixteen Area Agencies on Aging across Indiana.  The Indiana Association of Area Agencies on Aging, the Indiana Minority Health Coalition, and the University of Indianapolis Center for Aging and Community are partners on this grant.
