Six National Outcomes:

  • Families of children with special health care needs (CSHCN) will partner in decision making at all levels, and will be satisfied with the services they receive.
  • All children with special health care needs will receive coordinated ongoing comprehensive care within a medical home.
  • All families of children with special health care needs will have adequate private and/or public insurance to pay for the services they need.
  • Services for children with special health care needs and their families will be organized in ways that families can use them easily.
  • All children will be screened early and continuously for special health care needs.
  • All youth with special health care needs will receive the services necessary to make appropriate transitions to adult health care, work, and independence.

Prevalence in Indiana:

  • The prevalence rate of children and youth in Indiana who have limitations in functioning is 14.2% while the national prevalence rate is 12.8%.
  • Gender differences are apparent. In Indiana, 17.3% of the males ages 0-17 years have special health care needs when compared to only 11% of the females.
  • By race, 14.5% of the Whites, 13.8% Blacks, 10.1% of the Hispanics and 15.2% of the Multiracial ages 0-17 years old in Indiana have special health care needs.
  • Among CSHCN, 23.1% of children are between 0-5 years of age, 36.7% of children are between 6-11 years of age, and 40.2% of children are between 12-17 years of age. The majority of children with special health care needs are males (62.3%).

CSHCN National Core Outcomes:

  • The following is the table that summarizes the six national outcomes for CSHCN and the extent to which these outcomes have been achieved. Indiana is more successful in achieving the national outcomes when compared with the Nation (See Table).
