Needs Assessment Priority Survey

Every five years the ISDH MCSHC submits to the federal government a needs assessment for Indiana. The MCSHC services staff have identified the thirty-six issues listed below based on prevalence data and trends. We would like to have your opinion on which of these issues should be a main concern (priority) for the upcoming five years.

Please type the number of the issue you think is the most important in the left-most box (col 1). Then choose the two issues you think are of second importance (order doesn’t matter for these two) and place their numbers, one each, in the two boxes in column 2. Continue choosing issues and placing their numbers in the columns you feel they belong. Only one issue number can be entered into any one box, but you can change your mind any time and change numbers you have put in the boxes. Remember, issues within the same column have the same priority.

You can read a fact sheet that provides HP 2010 goals, data and trends about each issue by clicking on the issue. By clicking on “County Specific Fact Sheets” (below the issues list), you can also view county specific fact sheets. You may use these data to help determine your main concerns (priorities). After reviewing the fact sheets, close the fact sheet by hitting the “close” button.
