Your legislator will help you compile all of the necessary information you will need, along with providing a letter of recommendation of their own. You should have ready for your legislator the completed form, which is located below, along with some biographical information of the nominee’s life. The biographical information should including any highlights of the nominee’s life such as their service and/or other contributions to the local community.
